AltaMed is committed to prevention and early intervention of chronic disease by providing breast cancer screenings, childhood vaccinations, and consistent care for patients with diabetes and hypertension. Many of AltaMed’s patients are dealing with multiple health issues, while others are facing serious complications, including organ damage and risk of stroke, heart attack, and even death.

AltaMed will increase its outreach and community-building efforts, focusing on women and mothers, who often are responsible for their entire family’s health care. Our work with mothers will center on:

• Encouraging annual vaccinations and doctor’s visits.
• Prioritizing their own health screenings, including mammograms.
• Connecting them with a coordinated care provider who can help with follow-up and education.
